From remote consultation to follow-up at home

The document describes an overseas patient with multiple myeloma who consulted remotely, travelled to Shanghai for hospital assessment and T-cell collection, and later returned for inpatient treatment and CAR-T infusion, followed by overseas follow-up. Treatment eligibility, travel between visits and timing depend on the hospital's assessment of the individual patient.

Seven stages of continuous coordination.

Patient authorization comes first. Each step follows the hospital's formal advice.

  1. Initial enquiry and authorization

    Clarify the patient's needs, previous care, preferred language and local doctor's contact details. Explain the service scope and use of records, and obtain authorization before collecting or sharing them. Initial enquiries cannot establish eligibility or guarantee outcomes.

  2. Records and medical translation

    Organize records, reports and imaging as requested by the hospital, create a file index and coordinate translation. Flag unclear information for confirmation and submit the patient-confirmed version. The hospital determines whether more information is needed.

  3. Remote referral consultation

    Coordinate schedules for the patient, the Chinese medical team and the local doctor, arrange interpretation and collect questions. Doctors provide clinical assessment, risk explanations and advice; coordinators record administrative next steps.

  4. Hospital assessment and travel

    After the hospital confirms an appointment, coordinate arrival instructions, transport, accommodation and language support. Relay formal hospital feedback and outstanding cost questions. Update travel when plans change and distinguish hospital, coordination and travel charges.

  5. Between collection and the next visit

    Coordinate departure and a further visit only according to the hospital's formal arrangements, and confirm contact details for the patient and local team. The hospital decides whether travel home is appropriate, any monitoring required and when to return.

  6. Support during admission

    Provide interpretation, family communication and practical assistance. The hospital handles infusion, monitoring and adverse reactions. Report fever or other concerns immediately to the treating team; emergencies outside hospital require local emergency services.

  7. Discharge and overseas follow-up

    Help check discharge documents, follow-up appointments and emergency contacts, and arrange translations as needed. With patient authorization, relay formal hospital records to the overseas team. Medical teams remain responsible for tests, treatment changes and rehabilitation decisions.
