Privacy notice
This notice describes information handled by the current consultation form. Consultation records are used for intake, contact and care coordination.
Information collected
Identity and contact details, care goals, location, budget, timeframe, a diagnosis and treatment summary, and medical files you voluntarily provide.
Use and storage
Consultation records are stored in PostgreSQL on the application host. When uploads are enabled, files are stored privately in Alibaba Cloud OSS, currently in the Hong Kong, China region. The public site does not provide record downloads.
Access and retention
Authorized staff use the information for intake. Sharing with external institutions requires a separate explanation. Retention periods, privacy contacts and third-party arrangements must be finalized before public launch.
Your choices
Optional fields and attachments may be omitted. Avoid including unrelated third-party personal information. The production site must provide a way to request access, withdrawal and deletion.
